Whatever depth you want the market to have, plus the fee and gas. There is no minimum, but a pool with very little ETH in it moves violently on the first buy, and that is what the chart will show forever.
The ratio between the two amounts you deposit, and nothing else. A new pool has no market to reference, so whatever you put in is the price the first buyer pays. Work out the market cap that ratio implies before you sign.

Opening a range, not a curve
Cetus is concentrated, so creating a pool means choosing a price range as well as a price. A narrow range earns more fees while the market stays inside it and stops working entirely once the price leaves. Full range behaves like a classic AMM and is the sane default for a token that has not traded yet.
The ratio you choose is the price
A new pool has no market to reference, so the amounts you deposit are the opening price: put in 1,000,000 tokens against 1 ETH and the token opens at 0.000001 ETH. Get it wrong and the first trade is an arbitrage against you, which on mainnet is a mistake that costs gas to correct as well as value.
